Clinical Cases

Endodontic treatment of a calcified maxillary molar, the last tooth on the arch and pillar for a fixed prosthetic work. The 70-year-old patient, after performing a zirconium bridge, complains of spontaneous and irradiated pain towards the ear and the entire right half of the face. For the certainty of the endodontic diagnosis, we performed a computer tomography for endodontic purposes (CBCT) which confirmed that the starting point of the pain is the second maxillary molar (tooth 17), with a diagnosis of pulpal necrosis / chronic symptomatic apical periodontitis.
We achieved minimally invasive access through a zirconium bridge, we identified calcified root canals under microscopic control and we performed endodontic treatment in a single session (approx. 1h 30min). The patient reports the remission of painful symptoms shortly after performing the canal treatment.

Healing of an endodontic infection (chronic symptomatic apical periodontitis) highlighted on control radiographs 1 year after the endodontic treatment under a microscope. The initial radiograph, at the patient's presentation in the office, is shown in picture 1. Clinically, the patient shows abscess, spontaneous and irradiated pain, pain caused by touching the tooth and chewing food. The area of bone demineralization surrounding the top of the root of the upper lateral incisor (tooth 1.2) is suggestive of an endodontic infection (symptomatic chronic apical periodontitis / periapical granuloma). The tooth has been treated on the canal in the past and has a pin (dentatus) anchored to the root.
The treatment plan consisted of endodontic retreatment under microscopic control and corono-radicular reconstruction with fiberglass pivot, subsequently making a temporary crown of composite with which the patient remained during the monitoring of endodontic healing (1 year).

The control radiograph made 6 months after the end of the endodontic retreatment under a microscope is shown in picture 3. There are signs of healing and remineralization of the bone surrounding the top of the root of the upper lateral incisor. The patient no longer has any unpleasant symptoms and can comfortably use the tooth in chewing.
